Advanced Primary Care Management (APCM) is a proactive program designed to help you stay connected, supported, and organized between office visits. The goal of APCM is to make managing your health easier, reduce stress, and help you stay on track with your personal health goals.

If you live with conditions such as diabetes, high blood pressure, heart disease, COPD, kidney disease, anxiety, depression, dementia, or multiple chronic conditions, APCM provides an extra layer of care and attention tailored specifically to your needs.

What Is APCM?

APCM provides monthly support, care coordination, and personalized guidance to help patients manage chronic conditions more effectively. This service strengthens communication between patients and their AAMC care team and ensures your care plan stays current, proactive, and aligned with your goals.

Who Is Eligible?

APCM is ideal for patients diagnosed with one or more of the following chronic conditions:

  • Diabetes or pre-diabetes
  • High blood pressure
  • Heart conditions
  • COPD or asthma
  • Chronic kidney disease
  • Memory loss or dementia
  • Anxiety, depression, or stress

How APCM Works

Once enrolled, you will be connected with a dedicated Care Partner who stays in touch regularly and works closely with your primary care provider. APCM helps “connect the dots” between visits so you can spend more time doing what you love — with peace of mind knowing your care team is supporting you each month.

Your Care Partner will assist with:

  • Reviewing your health progress
  • Coordinating care with specialists or pharmacies
  • Updating your care plan
  • Providing reminders and helpful follow-ups
